All fees are due at the time of service.  

 

Please provide at least 24 hours notice for any cancellations or changes. If you do not attend your scheduled appointment and you have not provided notification at least 24 hours in advance, you will be required to pay the full cost of the session.

 

I am not a participating provider with any insurance company. If you would like to use out-of-network (PPO) benefits, the necessary paperwork will be provided to you to file a reimbursement claim through your insurance.
